Some of the modern armors are getting pretty good at handling rifle fire.
 
A .50 caliber heavy machine gun/sniper round is still a world of hurt to anyone hit, but .30 caliber is becoming a survivable possibility.

a sword or club will penetrate ballistic armor despite the fact that it can stop a bullet.
depends on the body armor. My IBA will stop a knife or club to the plate. i would still feel the impact, however it would be spread over the full surface of my torso and wouldnt do enough damage to phase me
and you could also add a new armor type:Ballistic which has a normal AT then gets a bonus AT vs projectiles.
yeah, if a 50 passes within a foot of you it can still rip off your arm
