If i give Marchosias his optional leader ability, can i give him an advantage card to?!?

Yes, that card with all the optional leader rules should be seen as a second character card like Cordy, Yuri, Kerberos, and many others have.  Its not considered an Advantage Card.

Another question: when Elienai takes damage from saving a character, can Sarah Reinhold take the damage instead as a reaction?

Yes the damage Elienai suffers is self-inflicted sacrifice damage so Sarah can take it instead of her.  It was likely designed for that very purpose.  But Sarah can also take the damage for Alis's Wings of Light, Romeo Pray token, Nero's Fallen, Xavier's Primordial Sin, Tsubasa's Ennetsu Jigoku, heck for giggles even Khaine's Martyrs Sacrifice.

The only one I'm not sure about is Cennette's team ability Live Blood
